Eligibility
Who Qualifies for Adult Foster Care?

Our program is designed for individuals who need daily support and want to remain in a safe, home‑based environment. To qualify for Adult Foster Care at Prestige AFC, members must meet the following criteria:

Basic Requirements

  • 16 years or older

  • MassHealth coverage or be eligible for it 

 

Care Needs

Individuals must have a diagnosed medical or mental condition that requires hands‑on help or supervision with at least one Activity of Daily Living (ADL), such as:

  • Walking

  • Eating

  • Dressing

  • Using the bathroom

  • Transferring (e.g., bed to chair)

 

Clinical Approval must be confirmed by both a Primary Care Physician (PCP) and MassHealth.
